Description *If a form is needed - this MAFH Health History form can be used!

For those needing a current wellness or school sports physical: The primary doctors wellness check-up or school sport physical form is acceptable. A physical must be within the calendar year of your camp session. (ex. Sports Physical 8/1/23 would qualify your child as cleared for camp; Sports Physical 7/1/23 would not qualify your child as cleared for camp.) Please contact with any questions or concerns.
